Activity Form
This form is designed for any ministry or committee to fill out for programs or special activities.
All activities must be approved before planning is done, to ensure that there is not a conflict with the calendar of events, and in accordance with FBC Ministries.

Guest Speaker Form
This form is designed to assist the FBC Ministry to better prepare for all who minister the gospel for whatever program or service it may require.

This form is for the sole use of the FBC Ministry. All ministries are required to comply.
FACILITY CONTRACT
This contract is made available for use of the Fellowship Hall for limited occassions.
The date and time must be provided so that we will not book anyone that conflict with FBC calendar of events.
FURNERAL/WAKES CONTRACTS
All payments must be withcash, a cashiers check or money order - personal checks will not be accepted.
